What do I need to bring for Live Scan fingerprinting?
You must bring a valid government-issued photo ID (driver's license, passport, or state ID) and the completed Live Scan request form provided by the agency requiring your fingerprints. The form must include the correct agency codes. If you're unsure, contact the requesting agency for the proper form before your visit.
